Please print and fill out our Complete Participation Packet or Event Packet and bring with you on your first visit.

PLEASE NOTE:

If participant is under 18 years of age or has a legal guardian: All adults who have legal custody are required to read, understand, and sign all consent forms in the ADVEC participation or event packet.

A volunteer staff member will sit down with you on your first visit to review the paperwork.
